Grief and Loss

Data:
Client attended a 60-minute session at the therapist's office. She communicated experiencing deep sadness and guilt due to the recent loss of her mother and not being present in her final moments. Client stated, "I keep thinking I should have done more." EMDR techniques were utilized to help her process these traumatic memories. Client appeared engaged with the interventions and demonstrated willingness to apply techniques discussed.

Assessment:
Client's presentation indicated active grieving and potential complicated bereavement. Her engagement in EMDR suggests a readiness to process her loss and guilt. Despite the difficult emotions, client's willingness to participate in therapeutic activities, like writing a letter to her mother, signifies positive steps towards coping.

Plan:
Client was assigned the task of writing a letter to her mother to express her feelings, as homework to support the processing of grief. The next session is scheduled for two weeks from the current date, at the therapist's office, to continue with EMDR and review the client's progress with the assigned task.

Client had a 50-minute session today discussing the recent loss of her father. She expressed deep sadness and difficulty accepting the reality. She said, "I miss him so much." We used EMDR to process her grief and explore her memories. Client’s homework is to create a memory box with items that remind her of her father. She appeared comforted by the process. Our next session is scheduled for two weeks from today.
