Note Instructions
Here's how you can generate a therapy progress note in just a few steps with Quill.
- Record or type a summary of your therapy session. (Details are included below of what you could include.)
- Click "Generate Note".
- Quill will generate a note based on the details you've provided. After reviewing, copy-and-paste that note into the EHR of your choice.
When summarizing your session with Quill to generate a therapy note, consider including some of the following details:
- Session Duration and Location: Specify how long the session lasted and where it occurred (e.g., virtual/telehealth or in-office).
- Reason for Visit and Chief Complaint/Presenting Problem: Describe why the client attended the session and their main concerns.
- Client Presentation and Mental Status: Note the client’s appearance, demeanor, and mental status during the session.
- Direct Quotes from the Client: Include notable statements (e.g., "They said, quote, 'I'm feeling more confident lately,' unquote").
- Reported Symptoms or Events: Summarize symptoms or significant events the client reported since the last session.
- Topics Discussed: Highlight key topics the client brought up during the session.
- Screening Results and Interventions: Document any assessments conducted and interventions applied during the session.
- Client’s Response and Progress: Describe how the client responded to interventions and their progress toward treatment goals.
- Changes to Treatment Goals or Diagnosis: Note any updates or adjustments to the treatment plan or diagnosis.
- Homework and Referrals: Detail any homework assignments given or referrals provided to the client.
- Planned Third-Party Contact: Mention any upcoming consultations or communication with third parties.
- Next Session Details: Include the date, time, and location of the next session.
You don’t need to include every detail listed above. Use this guide as a reference to help you rattle off a summary of your therapy session. Quill will adapt to the information you provide to generate a clear and well-structured therapy note.
Direct Quotes
To include a direct quote from a client, say "quote", then the quote, and then "unquote". For example: "Client said that they were feeling quote overwhelmed and unhappy unquote."
