Subjective:
Client reported increased panic attacks affecting her work and personal life. These episodes include sensations of impending doom and difficulty breathing. Client stated, "It feels like I’m going to die every time it happens." Discussed potential triggers and management techniques during the session.

Objective:
30-minute virtual session conducted. Client appeared receptive and eager to learn management strategies. Introduced deep breathing and grounding exercises as interventions during the session.

Client is experiencing severe panic episodes impacting daily functioning. Client is currently learning and seems amenable to implementing new techniques to manage panic attacks.

Client's homework includes practicing deep breathing exercises twice daily. Scheduled the next virtual session for two weeks from today.
